Nathaniel Borenstein, inventor do MIME, em: Programming as if People Mattered:Friendly Programs, Software Engineering and Other Noble Delusions, Princeton University Press, Princeton, NJ, 1991.

"The most likely way for the world to be destroyed, as most experts agree, is by accident. That's where we come in. We are programming professionals. We cause accidents. "
Nathaniel Borenstein, MIME inventor in: Programming the People Mattered if: Friendly Programs, Software Engineering and Other Delusions Noble, Princeton University Press, Princeton, NJ, 1991.

On February 25, 1991 in the first Gulf War, next to a province in Dharan, Saudi Arabia, one Scud missile hit a camp of the US army killing 28 US soldiers. The camp was protected by a Patriot missile defense system, which for some reason, failed to track and intercept the Scud missile. A year later, an investigation revealed that the problem in the Patriot occurred a fatal flaw in the arms of the battery control system: it was a mathematical failure. The time measured in the tenths of a second internal clock system is multiplied by 1/10 to obtain the time in seconds. The value of 1/10, which has an infinite binary expansion is truncated to 24 bits after the root point.
On the other hand, one of these Patriot missiles not reaching its target, fell close to another province, its warhead detonated not saving much of the population. In November 1992 there was a breakdown in the communication system of the ambulance service in London. In June 1993, for two days, was not allowed any credit card operation throughout France. All these defects were investigated and their particular causes, but do not have assurance that something similar can not happen again at any time development occurs in the mathematical idea fails as an area of research and thought, based on the term Fault Tolerance established by A.Avizienis lithuano in 1967. The term since then has been widely used by the academic community to describe the overall research area occupied by the behavior of computer systems subject to the occurrence of failures. The very term fault tolerance as area designation undergoes various criticisms, the largest of which is the ability to understand the term as an absolute property. In this view distorted or not, a fault tolerant system tolerate any failure in any situation.

Patriot missile system crashes may have killed / 22h10 - 02/20/2004
The United States Army has taken the Patriot missile system to Iraq before correcting its defects, the cause of the fall caused by "friendly fire" of British and American military aircraft, according to reports today, Friday, the CBS channel.
The television channel will broadcast a report on the subject, with interviews with researchers and weapons experts, including former Secretary of Defense Philip Coyle, on Sunday's "60 Minutes" program. According to the program, the Patriot missile system has flaws in computer systems that could have been the cause of the fall of a British Tornado fighter in March last year and an American F-18 in April, accidents that killed pilots
Another flaw occurred at the start of the war against Iraq when a battery of Patriot missiles hit a formation of four American F-16 fighters.
"I think military commanders knew about the problems, but they were so focused on firing on their targets that all other issues, such as the possibility of sparking a friendly fire, were not of equal importance," Coyle says.
The Pentagon confirmed that the missile system allowed the destruction of 10 missiles launched by the Iraqis during the war, but also produced three cases of "friendly fire".
"It is clear that failure to correct those mistakes that everyone knew existed 10 or 12 years ago have caused these unnecessary deaths," said gun specialist Joseph Cirincione in the report.
He added that "the system is a multimillion-dollar project that involves a lot of money and a lot of people's professional careers."
This is not the first time that this missile defense system has aroused concern and disappointment since important flaws were also recorded in the first Gulf War in 1991.
The Pentagon, the US Army and the Raytheon firm, which runs the Patriot missile system, declined to speak for the report, according to CBS.
